A property holder who has incurred exceptionally high expenses due to one of the following:

  1. A One-time or ongoing medical treatment of the applicant or a family member.

  2. A personal disaster that occurred to the applicant and which caused a significant deterioration in his financial situation. 

 

Applicants must fill out their details on the attached form.

An application that arrives at the collections department without all the necessary documents will be returned to the applicant until the missing documents are completed.
